The mistake is that the chuck will loosen whilst accelerating in reverse. Applying the brake whilst running in reverse will serve to tighten the chuck to the spindle.

You got it in seconds, also it would not be normal to have a spindle brake on a lathe with a threaded spindle for obvious reasons. I watched as the charge hand switched on a brand new Myford Super 7 and the 4" chuck unscrewed, bounced onto the bed, then the tray on its way to the floor. I was dissapointed that it had missed the fools foot. I had to stone the bed to remove the bruise. The chuck was only rotating slowly when it fell.

One must also be aware that inverters set to ramp down too quickly (possibly regenerating or using dynamic braking resistors) can cause a chuck to loosen when stopping while running forward.

                I missed all that.

All I could think was "Where can I get a hover-chuck like that one?"
