I already have a number of projects on the go and this is thinking ahead a bit :

I like this marine steam engine by PM Research. I was thinking of building some sort of freelance locomotive with it. The engine is 4.5'' wide. What gauge would be suitable for an engine of this size ?

I assume that this engine would be a self starter ie. I would not have to kick over the flywheel to get things moving ?

How about a 3ft long boat which is what it is designed for?
I know it's not what you asked.
If they do a similar sized in line it would do for a 2 1/2 in Sentinel.

One of our club members made a 5 in Sentinel with a Stuart D10 but I think it was a bit underpowered.

I don't see wht not. It would be better to turn the engine so the V runs front to back that way you don't need to turn the rotation through 90degrees.

A simple chain or gear train down to the driven axle can be done with off the shelf parts, Wheels and axles can be bought ready made or wait until you have a lathe, something like this maybe

I was thinking of a Heisler style locomotive with geared trucks ie. the V across the loco. I already have the necessary universal joints.

If I was going to go for an in-line engine then I would probably choose the TVR1A from Graham Industries :
